Scapholunate dissociation (SLD) is the most common type of carpal instability. It is the most common instability pattern because the intercarpal motion during flexion and extension cross at the scapholunate interface. Another reason is that compression forces transmitted through the capitate has a tendency to separate the scaphoid and lunate. Scapholunate dissociation results from an injury to the scapholunate interosseous ligament which joins the scaphoid and lunate wrist bones together. This ligament is often torn by a fall onto an outstretched hand. This can tear the ligament, causing the wrist bones to move apart or ‘dissociate’ from one another when the wrist is used. A grip compression view or wrist in ulnar deviation may be necessary in addition to PA/lateral views.
